2012 IQ rough idle when in drive or reverse

Two Toyota dealers can’t find the problem. One said it was the throttle body, it wasn’t, the other said I needed a new engine. The car runs great, has power and gets normal gas mileage. It all started after I drove down a dirt road and scooped up some rocks and things. Motor mounts were checked, local garage said it was #3 coil, it wasn’t. Has anyone had a similar problem? I bought this car new with factory lowering springs in 2012 .I now have 130K miles.

Maybe a vacuum leak? Using a scanner might tell you something. Does it get smoother as you drive away? Comes back rough when you stop at a light?
I’ve used a scanner but no results. The car runs great once I get going, it just idles rough when I put it in gear, really bad in reverse, almost like it wants to stall. I can’t see how this can be an engine problem since it runs great and has plenty of power.

Have you inspected the transmission wiring harness? There are multiple sensors around the K41B CVT. Lift it up and see where the rocks may have damaged it. Piccies of the transmission if you have time, please. ;)

Which scanner did you use? Some of the more modern/expensive ones allow you to perform active tests to the various systems. ;)

I will check the transmission sensors. As for the scanner I used one that I bought on the internet.
Here are the location of the 3 speed sensors on the K41B CVT. ;)

I checked the wiring to the sensors and see no problems.
With your scanner, are there any codes set or pending? When you say great gas mileage, it's still around 36mpg city? ;)

Any aftermarket products installed? Any other issues? Were there any issues with the air filter when you changed it? Did you pull the intake tube off and inspect the throttle body fer any issues? Where are you located near? If you are close to Houston, Texas, I could come by and check it out. I don't have a lot of faith in the dealerships when it comes to troubleshooting these days. ;)

Have you inspected/cleaned yer MAF sensor? With yer OBDII scanner, you should be able to monitor the MAF sensor. With the engine idling, it should read near 1.3 grams per second. ;)

You should be able to test the throttle position by removing the intake tube. Ignition on, engine off. With a helper, step on the gas pedal slowly and observe if the throttle body opens correctly. You should be able to also monitor the throttle body and status with the OBDII scanner. ;)

I’m using a cold air intake and replaced the filter, haven’t replaced the PVC. Is there a way to adjust the idle? My main concern is the rough idle when in gear, especially reverse, feels like the engine wants to stall. The ON scanner doesn’t show anything. Spark plugs were replaced along with the #4 coil. I’m perplexed because the car runs great and gas mileage is pretty good. I can live with the oil consumption problem for now, I might add that I do a lot of highway driving. I appreciate your interest and hope I can resolve this issue, I love this car so much I bought another one, a 2014 .

There isn't a way to manually adjust the idle as it's controlled by the ECU/ECM. Is the AC on when the issues happen? Have you tried it with the AC off and see if the issues still persists? ;)

I'm going to see if my XTools scanner will be able to give me some data fer you to work with like the MAF readings, TPS percentage at idle, etc. ;)
A new scan is saying smog check failure but doesn’t indicate what’s causing it. Starts up okay but when the engine warms up the rough idle begins, engine is idling at approximately 630 rpm. The VSC icon is lit and I can’t turn it off.
